Practice Policies & Patient Information
Complaints
We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.
To pursue a complaint please contact the practice manager who will deal with your concerns appropriately by filling out the form below.
Disabled access
There is good access for the disabled to all of our consultation rooms. A designated parking bay is adjacent to the surgery entrance and the surgery has a ramp and wide doors to allow for wheelchair access.
Named GP
As part of the commitment to more personalised care for patients, NHS Employers and the General Practitioners Committee of the British Medical Association have agreed that all patients will have a named accountable GP.
The named GP will:
- Take lead responsibility for ensuring that all appropriate services required under the contract with the practice are delivered to you
- Where required, based on the professional judgement of the ‘named’ GP, work with relevant associated health and social care professionals to deliver a multidisciplinary care package that meets your needs.
- Ensure that your physical and psychological needs are recognised and responded to by the relevant clinicians in the practice
- Ensure that patients over 75 years of age have access to a health check if requested, which is already a requirement of the GP contract regulations.
The practice will ensure that there is a named accountable GP assigned to each patient.
New patients will be allocated a GP at the time of registration.
Your named accountable GP will be the same as your usual GP however this does not affect your ability to see any GP of your choice as you currently do.
Privacy Notice
As data controllers, GPs have fair processing responsibilities under the Data Protection Act and GDPR law 2018. This means ensuring that your personal confidential data (PCD) is handled in ways that are safe, transparent and what you would reasonably expect.
This privacy policy sets out how Dr Shantir’s Practice uses your personal information.
Shared Care Agreements
From 1st October 2024, Dr Shantir Practice will no longer accept new Shared Care agreements.
We have made this difficult decision jointly with our neighbouring practices in Forest 8 and Walthamstow West Primary Care Networks.
We would like to explain to patients why we have had to make this decision.
Summary Care Record
Our practice is participating in NHS national programme for IT. We share summary of your vital clinical information i.e. Medication, Allergies and Adverse reaction to drugs, with the NHS Emergency Care Providers like A&E, Out Of Hours GP and Walk-in Centre. This is to ensure that our patients receive safer and quality care during urgent or emergency situations.
Zero Tolerance
It’s essential to treat all staff members with respect and courtesy. The practice maintains a zero-tolerance policy to ensure the safety and protection of both staff and patients.
